Description
Continuing Life is seeking a part-time (16-20 hours per week) Clinical Coach who will travel throughout California (in partnership with our full time Clinical Coach) to provide guidance and mentorship to clinical staff members, ensuring exceptional patient care outcomes. This position requires significant expertise with MDS. AANAC certification preferred.
This position can be based out of San Diego, Orange County, San Fernando Valley/Ventura County, or Pleasanton, California.
PRINCIPLE DUTIES:
- Maintains up to date knowledge of, and ensures compliance with, all Federal and State regulations pertaining to resident care in the SNF and RCFE levels of care.
- Participates in trouble-shooting with Director of Operations, Executive Directors, Directors of Nursing and Administrators to minimize risk.
- Supports the Assistant and Directors of Nursing with staff development, implementation of clinical protocols and the development/revision of policies and procedures to ensure compliance with regulations and current standards of practice.
- Audits nursing records and notes on an ongoing basis. Ensures compliance with record retention of such.
- Works with the clinical and administrative team in the screening of applicants for nurse management positions.
- Stays abreast of clinical trends and best practices throughout the senior living industry.
- Provides support as needed through the survey process including assistance with preparation for survey, writing the plan of correction, developing an action plan, and monitoring for compliance.
- Supports a dignified and caring atmosphere with residents, residents' families, visitors and staff. All information obtained in the field is kept strictly confidential.
- Assist with the implementation and maintain, audit and create action plans for electronic medical record system.
- Key administrator of electronic medical record system (Point Click Care).
- Serves as temporary interim DON during position vacancies or extended leaves of absence.
- Protects the privacy and confidentiality of information pertaining to the residents and employees.
QUALIFICATIONS:
- Minimum of five years as a Director of Nursing (DON) in a skilled nursing facility.
- BSN or MSN preferred
- Working knowledge of State and Federal regulations and the survey process.
- RCFE license preferred.
- Expert level knowledge of the Minimum Data Set (MDS) and Resident Assessment Instrument (RAI) process.
- Have a current driver's license and access to a personal car. Ability to work with a team, be self-directed, organized and flexible.
- Ability to travel up to 75% of time to locations in California, mainly Southern California.
- Excellent customer service skills.
- Excellent written and verbal communication skills, with the ability to facilitate group presentations.
- Strong organizational and time management skills. Strong attention to detail.
- Proficiency with technology, including Microsoft Office Suite & Point Click Care. Ability to learn new computer applications.
