Description

Cedar Park Med is hiring Licensed Practical Nurses (LPN) for immediate full-time openings in a long-term care (LTC) nursing home in Montrose, NY. This is a long-term assignment offering consistent scheduling, a supportive clinical environment, and paid orientation. There is a temp-to-perm opportunity if desired. You’ll have direct communication and ongoing support from our team. Start ASAP.

Shift / Schedule

• Evening: 3:00pm – 11:30pm
• Night: 11:00pm – 7:30am
• Full-time
• Long-term assignment
• Temp-to-perm available
• Weekend rotations included 

Position Overview

In this LTC setting, the LPN provides direct resident care under RN supervision while supporting safety, stability, and continuity of care. This role requires clinical judgment, accurate documentation, and strong collaboration with the interdisciplinary team in a structured long-term care environment.

Responsibilities

• Administer medications and treatments as ordered
• Monitor residents’ conditions and report changes to RN or provider
• Perform wound care and routine nursing procedures
• Assist in developing and updating individualized care plans
• Document care accurately and maintain compliance standards
• Collaborate with CNAs and healthcare team members to support resident outcomes

Required Qualifications

• Current NY LPN license
• Minimum 6 months of LTC experience
• Current BLS certification or ability to obtain prior to start
• Compassionate, resident-focused care approach
• Reliable attendance in a structured care environment
• Experience with medication administration in a nursing home setting
• Strong documentation and communication skills
• Ability to work effectively within a structured team environment
