Anchor Health is seeking a professional Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our team. The PMHNP will provide outpatient psychiatric evaluation and medication management services to the adolescent and adult patients of Anchor Health. The PMHNP will work in coordination with Anchor Health’s case management and medical staff to provide holistic and patient-centered care.

Organizational Snapshot: Anchor Health currently has a team of 28 employees, including 5 clinicians, 3 nurses, 5 medical assistants, 4 patient care associates, 2 case managers, 2 pharmacists, 2 pharmacy technicians, and a handful of administrative staff.
Responsibilities:
- Conducts psychiatric evaluations, diagnostic and risk assessments, and diagnosis of adolescent and adult patients.
- Develops and carries out a plan of care, including medication management, psychotherapy, counseling, education, and evaluates patient response.
- Collaborates with medical providers and nursing staff to ensure high quality, coordinated care.
- Provides outpatient referrals for additional behavioral health services such as psychotherapy or neuropsychology.
- Provides consultation and health education to providers and staff as it is relative to patient treatment and medication needs.
- Evaluates and writes letters of support for trans and gender diverse patients planning gender-affirming surgery.
- Issues and refills prescription medications in accordance with the plan of care, treatment guidelines, best practices.
- Responds to all refill request and patient messages within 3 business days.
- Completes comprehensive and accurate documentation in the electronic medical record (EMR) within 7 days from the date of service.
- Properly codes and bills outpatient psychiatric visits for medication management within 7 days from the date of service.
- Coordinates and communicates effectively with all parties involved in the care of the patient including, Anchor Health medical staff, patient family members, external clinicians, school system staff, and government agency employees, as applicable.
Requirements
The Ideal Candidate: The ideal candidate for this role is passionate about LGBTQ health care. Anchor Health is a busy and dynamic workplace, and they should have excellent communication, time management, flexibility, and critical thinking skills.
Requirements:
- Current Psychiatric-Mental Health Nurse Practitioner (PMHNP) board certification from the American Nurses Credentialing Center (ANCC).
- An unrestricted license to practice as an Advance Practice Registered Nurse (APRN) in the state of Connecticut.
- A valid and unrestricted DEA license to prescribe controlled substances.
- An unrestricted Controlled Substance Registration for Practitioners (CSP) in the state of Connecticut.
- No suspensions, exclusions, or other prohibitions from participation in federal health care programs (e.g., Medicare, Medicaid, SCHIP).
- 1 year of experience providing psychiatric services to patients from the LGBTQ community and awareness of needs specific to this population.
- 2 years of experience providing psychiatric services as an APRN.
Preferred requirements:
- Spanish language fluency.
- Experience using the EMR, Epic.
- Understanding of and support for the transgender community.
Compensation and Benefits: This is a full-time, exempt position. Part-time is a possibility, but full-time is preferred. The expected salary range is between $130,000 to $140,000 annually, depending on qualifications and experience.
