Description
Under a physician-approved plan of care, the Occupational Therapist helps patients achieve their highest level of independence and functional ability through the application of occupational therapy principles and practices. The therapist evaluates patient needs, develops and implements treatment plans, and collaborates with the healthcare team to support optimal recovery and quality of life. The role also includes educating patients, families, and caregivers, as well as assessing home environments and recommending modifications to ensure safety and independence.
Key Responsibilities
- Evaluate patient's physical, cognitive, and functional abilities
- Develop and implement individualized occupational therapy treatment plans
- Assist patients in improving daily living skills and functional independence
- Provide consultation and collaboration with physicians and other healthcare team members
- Educate patients, family members, and caregivers on therapy techniques and safety practices
- Assess home environments and recommend modifications or adaptive equipment to enhance safety and independence
- Document patient progress and update treatment plans as needed
Note:
Remote: Clinical staff travel to patient's home to complete visits.
Requirements
- Current California Occupational Therapist (OT) license
- Minimum of one year of recent experience in an acute care, skilled nursing facility (SNF), or home health setting
- Strong clinical assessment and treatment planning skills
- Effective communication and collaboration with patients, families, and healthcare teams
- Ability to work independently and manage multiple patient cases.
