- Local visits and timepoints are available within approximately 15 miles of the towns listed above.

We are dedicated to providing compassionate, high-quality clinical home care across Connecticut. We are actively recruiting experienced and compassionate RNs and LPNs to expand our in-home care team.

What We Provide:

Requirements:

• A current, active RN or LPN license in Connecticut
• Valid CPR certification
• Driver's license and reliable transportation
• Strong clinical judgment, communication, organizational, and time-management skills
• Comfortable with technology and electronic health records
• Previous home health experience preferred but not required
• Familiarity with Medicaid, Medicare, and Sandata EVV is a plus
• Bilingual Spanish-speaking clinicians encouraged to apply - salary incentives available

Key Responsibilities: (Depending on licensure)

• Perform complete patient assessments and develop individualized, evidence-based care plans
• Reassess and update care plans as patient needs change
• Provide hands-on nursing care, administer medications safely, and educate patients and family members
• Coordinate care with interdisciplinary team members and community resources
• For RNs: manage an assigned caseload as the primary case manager and oversee clinical outcomes
